Yes. The form page signs the visitor in with Discord before it shows any question, which is what allows one response per person, membership checks and ban appeals tied to a real ban.
Yes. Building forms, collecting responses and reading the per-question statistics work on every plan. Premium adds CSV export, custom branding on the form page and reviewer roles, and raises the number of active forms.
No, only active forms count. You still need to be under the limit to create a form, and switching a live form off does not free its slot; deleting it, or letting it reach its closing date, does.
