Applications, feedback, ban appeals: build the form on the dashboard, share its link and collect the answers.

Ten question types

Short and long text, single choice, checkboxes, dropdown, rating scale, number, email or URL, date and date range, each with its own validation rules.

Conditions and branching

Show a question only when an earlier answer matches, make one required only in some cases, and send people to a different section from the option they pick.

Review from Discord or the dashboard

Each submission is posted to a staff channel with Accept, Deny and Dismiss buttons, and a decision can add roles, remove roles and DM the respondent.

Ban appeals

A ban appeal form is linked straight from the ban DM, with a waiting period, a cooldown between attempts and a blacklist for anyone who abuses it.

Questions about Forms

Does answering a form need a Discord account?

Yes. The form page signs the visitor in with Discord before it shows any question, which is what allows one response per person, membership checks and ban appeals tied to a real ban.

Is Forms free?

Yes. Building forms, collecting responses and reading the per-question statistics work on every plan. Premium adds CSV export, custom branding on the form page and reviewer roles, and raises the number of active forms.

Does a draft form count toward the limit?

No, only active forms count. You still need to be under the limit to create a form, and switching a live form off does not free its slot; deleting it, or letting it reach its closing date, does.
